having just completed it, I'd have to put a vote in for the oil pan seal.
would never of had to do it if I hadn't dropped something in the motor

things that made it a royal PITA:
did it with engine/tranny in car (mistake #1, IMHO)
had to remove passenger header (longtube was in the way of starter removal and flywheel housing and was being a general annoyance)
starter came off
had to jack motor up, put blocks where the motor mounts were
motor still wasn't high enough to clear the steering shaft/gear thingy that runs across the k-member,
so the steering thingy had to come off too
then of course I had to put everything back.
Of no surprise to me I had misplaced two of the oil pan bolts, so I had to snag a few from the junkyard to complete it.

all told... the labor must be a $400 job with a $15 gasket I guess.

son of a FReaKING$@$! wHORE!#%@$

other favorites:
heater box
rear main seal

I'm sure there are others...

Regular maintenance?

I hate adjusting valves on an EFI intake 5.0 or if you have ever had poly locks back off

Try something as simple as replacing a PCV valve on a gen 1 Lightning . Gotta pull the FU**IN upper intake to access it. Ford couldn't put it on the valve cover [BS flag] . NICE design..

Even changing the plugs the first time on the 4.6 motor is a PITA for such a simple friggin thing. Once you have done it a couple times though, you get the rythem.

you think that's tough... try changing plugs on an LT1. Yes, I'm talking about the ones under the dash!
