First get a new gas cap to pass the pressure test..Also the numbers look excessively high and looks like you have a bad catalytic converter...

I knew about the gas cap instantly. I figured it was a cat. Would you suggest changing spark plugs and 02 sensor or just the cat?

If you haven't replaced the spark plugs it's a good idea for maintanance.It if was me I would also replace the oxygen sensor for better readings after installing a new catalytic converter...

do a compression test, i dont think its missing bad enough for a miss fire, but it is not running right, an its only slightly off. if your compression test passes, then start tuning it up.

With Nox numbers that high it seems like the egr valve is not working or the passage way has become partially blocked you need to have it cleared out I had the same problem on my Honda civic

Seems to me that the egr is not functioning properly or the passage has become blocked and needs to be cleaned out I had the same problem on my civic and it kept failing smog until I had the egr passage cleaned on my intake then numbers dropped dramatically

Yea I'd check the egr system those nox numbers are high. Some of those Hondas have passages in the intake that get clogged. Then I'd tune it up and make sure to go to a pass or retest free place lol I it still doesn't pass throw a cat on there .

change the cat and the front oxygen sensor its best to change them together because a bad o2 could have made your old cat go bad due to excess fuel.also there is no egr valve so your emmisions control is strictly off your 02 sensor and your cat.i see this everyday
