What mods are done to the car?

Old cat converters...timing is noted at 9° it should be 10° also can have vacuum leaks...check and squeeze all the vacuum hoses for cracking. Even the vacuum caps look fine but show cracking or deterioration if you gently squeeze these items. Evap off the intake is a common over looked items well. Dirty egr....causes an internal vacuum leak. Pcv valve grommet gets rock hard and needs to be soft and plyable to seal correctly.

At a glance I would say bad Cats for sure. Other problems like vacuum leaks in my experience wouldn't cause that bad of emmisions.

looks like the guy ran it in 2nd gear for the 15mph and then kept it in 2nd for the 25mph, BAR says if the engine being tested is greater than 3.0l you can test in 1st and then shift to 2nd for 25mph, i bet you if you get those cats nice and warm and have him test it in 1st for 15 then 2nd for 25 it will pass....

Do your headers have a carb plate on them, pullys work your motor alot more at any rpm and cuts your mortor life in half. And just the cai alone shouldnt pass visual.
